Program Description
Polk County Home Care has provided a seamless continuum of quality health care services to people of all ages, since 1967, in the comfort and safety of their own homes. Our highly skilled health care professionals offer a wide variety of home health services and are dedicated to help Polk County citizens maintain their independence, improve their health and enhance their quality of life.

• Skilled Nursing Services

  • Observation & assessment of the patient's condition

  • Skilled procedures to treat illness or injury - IV therapy, complex wound cares

  • Provide health care management with HomMed Monitoring System

  • Teaching and training patient or family how to manage treatments & cares

  • Management & evaluation of patient care plan-case management

  • Medication Management

  • Modified hospice cares

• Home Health Aide/Personal Care Worker (PCW)

  • Hands-on personal care including bathing, dressing and grooming

  • Assisting with exercises as set up by physical therapy

• Therapies

  • Physical Therapy

  • Speech Therapy

  • Occupational Therapy
