Goal:
To help women have healthy pregnancies and healthy babies.
Services Provided
-
Education to high-risk
pregnant women on: self-care, fetal growth and development, smoking
cessation, nutrition, preparation for labor and delivery and
substance abuse
-
Referral for financial
counseling, housing, nutrition and school completion
-
Postpartum education
and referral on self care and recovery, infant care and feeding,
family planning, well child care, immunizations and parenting
Five Core Services of
Prenatal Care Coordination

Conduct outreach
activities to help MA eligible women connect with the Prenatal Care
Coordination program

Provide an assessment
for the woman which identifies important information regarding the
woman's medical, social, emotional, nutritional and educational
needs
-
Develop a plan with
the woman that identifies the services she needs to help her have a
healthy birth
-
Provide services,
coordinate services and/or refer women to the services they need.

Follow up with clients
to ensure they are receiving necessary prenatal, postpartum and
newborn services
Cost:
Billable to MA or no cost.
