Program Description
School Health promotes health and well being to Polk County's school age population by following these objectives:

• Access to appropriate health-related services and resources for school-aged children and their families

• Health education/information for children, school staff and families

• Prevention of vaccine preventable disease through optimal levels of immunizations for children and staff

• Early identification of hearing problems to reduce learning difficulties and possible long-term hearing loss

Services Provided

• Consultation to schools and families regarding the Wisconsin School Immunization Law and recommended vaccinations for school-aged children

• Coordinate special vaccination clinics in schools for age-appropriate students and staff

• Collaboration with schools to conduct annual school-based hearing screening program

• Participation as requested in multi-disciplinary team meetings for students with special needs

• Consultation and referral for families and school staff regarding health-related issues for individual students

• Education/training to school staff as requested by physicians regarding individual medical needs/treatments/procedures for students

• Classroom presentations on health-related subjects

Cost:

Polk County Health Department contracts with individual school districts and private schools to provide school nursing services. There are no costs to individual students or families.

Services to schools may vary depending on whether a school district has it's own nurse, the size of the school, and/or the needs of a particular student population.
