The PCHD strives to regularly communicate with its partners and the public. For organizational purposes, communication is broken down into two categories; external communication, and agency guiding policies, plans, or procedures. Here is a short description of each:

  1. External Communication: The primary way to communicate externally is through Press Releases. The PCHD often communicates important information, events, recommendations and agency events to the public via the media in the form of Press Releases. These Press Releases appear in local and county-wide newspapers and are archived on the link to the left named 'Press Releases'.
  2. Agency Policies, Plans or Procedures: These often larger documents are the driving forces of the PCHD. They guide how the agency will move forward and continue to provide the highest quality programs and services to the people of Polk County. Some examples of these documents are;
    • Strategic Plan: This is a 3 year work plan that guides many of the decisions of the health department
    • Annual Reports: These are required reports that provide a snapshot of what the health department has done throughout the year
    • Community Health Assessment: This is a comprehensive assessment that identifies the top health priorities of Polk County and the process used to determine those priorities
    • Community Health Improvement Plan: This is a comprehensive 5 year plan that addresses how the PCHD, its partners, and the county will work to address the health priorities identified in the Community Health Assessment
