Reproductive Health

Polk County Reproductive Health Services provides confidential, cost effective, basic reproductive health services to both female and male clients. Our program strives to reduce unintended pregnancy and sexually transmitted infections, and to promote early identification of pregnancy.

Services

  • Contraception and Emergency Contraception
  • Sexually transmitted infection testing, vaginal infection testing and HIV testing
  • Physical examinations including breast exams and cervical cancer screening     (Pap tests)
  • Pregnancy testing
  • Access to HPV vaccine for those who qualify

Cost

Sliding Fee Scale based on income or Family Planning Only Services (FPOS) if you qualify. Follow this link for FPOS service coverage and information on how to apply.

First Appointment

Please show up 15 minutes early to allow time to fill out forms. Also please bring with you:

  • A photo ID
  • Birth Certificate- If born out of Wisconsin
  • If 18 and older- Proof of income for the last 30 days
  • If you are covered by Badgercare Plus or Badgercare Family Planning, we can bill those services. Please bring your card if possible

Directions- Park in the south parking lot. Once you are in the building walk straight towards the rear of the building to the Health Department. You will pass an elevator on your left. Enter the first door on your left after the elevator. This door leads you to the Public Health reception area. Reproductive Health will be straight ahead.
