Polk County Reproductive Health Services provides confidential, cost effective, basic reproductive health services to both female and male clients. Our program strives to reduce unintended pregnancy and sexually transmitted infections, and to promote early identification of pregnancy.

To save time at your appointment, click on the appropriate Health History form below. Simply print it, fill it out and bring it in to your office visit.
Directions- Park in the south parking lot. Once you are in the building walk straight towards the rear of the building to the Health Department. You will pass an elevator on your left. Enter the first door on your left after the elevator. This door leads you to the Public Health reception area. Reproductive Health will be straight ahead.
